What are the most reliable KPIs to measure the success of an ongoing Agile transformation?

You need to move away from "output" metrics and focus on "outcome" and "flow" metrics. Lead Time (from idea to production) and Cycle Time (from work start to finish) are the most objective measures of agility. If your transformation is working, these numbers should be trending downward. Additionally, track "Quality" through escaped defect rates and "Value" through actual ROI of the features delivered. When you show the C-suite that you are delivering features 30% faster with 20% fewer bugs, they won't care about subjective happiness scores. These metrics provide a clear picture of efficiency and reliability.

Focus on "Throughput." It’s much harder to game than Velocity and gives a clearer picture of how many items are actually reaching the customer.
