It’s a common misconception that Six Sigma is too slow. You don't apply DMAIC to every Sprint; you apply it to the process of how Sprints are run. Use the "Measure" phase to track your Defect Density and "Analyze" to see if bugs are coming from specific modules or lack of automated testing. We found that our "Sigma Level" for code quality was low because our peer review process was inconsistent. We used Six Sigma to standardize the review checklist, and our production bugs dropped by 30%.

How are you defining a "defect" in your data? Are you counting UI tweaks as bugs, or only functional failures? Defining your "Critical to Quality" (CTQ) metrics is the first step.

• Walter, that's a fair point. Currently, we lump everything into Jira as a "Bug." Using a CTQ tree would definitely help us separate "Critical Failures" from "Minor Enhancements." This would make our data much cleaner for analysis and help us focus on the errors that actually impact the end-user experience.
